Mid 2015 15" r-MBP

I just did a clean install of 10.10.5 (CMD-R restart, Disk Utility, erase partition, download and reinstall Yosemite)

Now, even though I have a set a password for my user account, when I restart, I have to first enter iTunes password on the log-in screen, then it goes again to the startup screen where I enter the account password. What is going on?

I would suggest looking in the console log app logs for a crash log that has a time/date stamp around when the issue happened.

Actually, so I log in at login screen after a restart, log in screen has only my account and guest account, log-in with icloud password, and the progress bar goes about a third of the way, then screen flashes black with a white apple, then the progress bar finishes, and then I go back to the same login screen with my account and guest account, and then this second time, I use my regular account password.

WTH?

even if I log out of iCloud, I still have the same issue restarting. Anyway to do anything more drastic than a clean install, again? I don't mind doing it and probably will, but anything even more permanent?

WTH?

This sounds like the window server or another process has crashed. This might be caused by a third driver crashing. I had a situation where a Wacom tablet driver was crashing and caused a similar issue. I uninstalled the Wacom driver and the problem stopped. Look for an entry in the console app that ends with ".crash" to find the crash log entries.

EDIT: Before you go doing a clean install see if you can find a crash log. It might tell you what might be causing the crashing issue. Which might lead to uninstalling a driver or program.

There's literally nothing installed is the crazy part, I just did the format and clean install today!

How do I read the log to find a crash?

In the console app you click on the entry and it shows you the log. You can copy the text and paste in between QUOTE and /QUOTE tags. In other words I have put an extra space around "QUOTE" and "/QUOTE" which you won't do.

Well, it's resolved. The first clean install involved my deleting the partition and then downloading in the install OS X section. My second install resolved the issue by booting with removable key.
