I'm trying to play an old PC game on my Mac (10.5.8). I've installed MacPorts, and I've installed Wine... now I'm in the MacPort terminal and I'm the right directory for the game, however when I attempted to install and/or run the game (via the terminal), I get this response: "16-bit/DOS support missing."

Is there any way to adjust and fix this?

custom build Wine yourself without disabling win16 .... macports does that depending on your machine and compiler cuz there are major issues with it building on some machines.

If you want to use Wineskin, the WS3Wine1.2RC7 engine I have up has win16 built in... and should work on any 10.4+ machine.
