Do I need to flash any firmware to run two X5690s in a 2010 Mac Pro that has a CPU card from a 2009 Mac Pro, or is it plug and play?

Physically it will fit, but the fans will run at top speed. Also the 2009 cMP CPU trays use unlidded CPUs. Have your CPUs been delidded? Or are you planning on using spacers to mount your CPUs?

Basically a mismatch that you should not contemplate.

If he swap the trays SMC chip, will fans back to normal?

No clue if that works, if able to program it with the newer version then it would work. But no one has ever done that I have hear of and if it were possible they would be doing it for certain and we would have known about it by now.
