Having had a similar occurrence a few weeks ago, I'm fairly confident that the video ram on your graphics card has or is failing. Suggest you look on eBay for a used replacement and install it yourself as this should be much cheaper than buying a new card off Apple.

Problem was the card. Apple store replaced the card with next-day part and no service charge even though the Apple Care was 6 months expired. Great service from the local Apple store [Jax].
