iPhone 3gs unlock problem - no service after unlock

have a newer bot 3gs, previously JB on 4.1.. & unlocked with 06.15.00 BB, unlock was working fine.
restored via itunes to 6.0, then JB with redsnow, installed ultrasnow & ultrasnow fixer, uninstalled and reinstalled them in correct order, still showing no service with all sims.
called up provider and got factory imei unlock done, this has been confirmed by apple as being unlocked, still showing no service with all sims.

what do i have to do to get my unlock back ?

What are the first five digits of the serial number? I heard the factory unlock will not work with the iPad baseband.
