Activity app notifications?!

Ever since the beginning of iOS 10 each time I turn-off (disable) Activity app notifications it re-enables after a reboot of the iPhone. Badges, Sounds, Banners.

Why is this?

No, I do not have an Apple Watch.

The only place it shows up is in notifications.

Weird. It's never shown up for me.
