Hi,
I recently purchased a Developer Account to be able to upload apps to the App Store, and I moved from the Public Beta of iOS 9 to the Developer Preview. I noted that I no longer have the feedback app. I reinstalled the beta twice now and still have no avail.

Other notes:
iPhone 6
Apple ID for the iPhone is not the same as the developer ID(this is because I want to keep my personal Apple ID operate form the developer one)

The feedback app comes with the profile required to install for the public beta program. Just install the profile again if you really want the app back. But you'll have to remove the profile again if you want to OTA further developer builds. (Profile only looks for PB builds OTA.)

So as a developer, I have to submit them online? That's fine by me if so.

Since the DP and PB have the same build numbers, install the beta profile and submit reports through the Feedback app. When a new DP comes out, delete the profile, reboot, and check for an update.
