Do you have to use both for the mic to actually work better ? I’ve had callers saying they cannot hear me well when I’m just using one side.

No only one of the mics work even when both pods are in use. You may have a faulty pair. also try switching them use one and see and then the other. The mics are very good on the AirPods, I have had people not even know I was using an earpiece.

Or could it be the way I’m wearing it? Should I wear as in the Apple commercials? With the tip facing my mouth ?

That could be it also. When I put them in I give them a little twist forward so yeah mine are more directed towards my mouth.

Try different positions and see if it makes a difference. I know when I would try to adjust my AirPods, I found Siri responded much better when I had the tips facing straight down versus on an angle.

My problem is the person on the other end hears everything around me. People complain it's difficult to hear me over the background noise. I can't take a call in an airport or somewhere in public because of it.

Does anyone else have this problem?

Using my Airpods with an iPhone 7, folks complain that I sound muffled. I haven't seen reports of this from other users. Is this normal, or do I need to check with Apple?
