I'm pretty sure this issue wasn't present before the last iOS update, but is anyone else experiencing the following?

When the Off setting is ticked in the bluetooth settings on the iPhone for the AirPods Pro or AirPods Max.
Randomly, the option unticks itself. Which means when cycling through the noise cancelling options on the mentioned earphones/headphones, the only two options are Transparency, Noise Cancellation, until I go back into Settings/Bluetooth (and press the i next to the AirPods Pro, AirPods Max) and ticking the Off option again in the "Button Cycles Between" section.

I've tried doing a factory reset, but it makes no difference. I suspect it's a software bug, as this issue hasn't always been present, and I was just curious to hear if anyone else is experiencing this issue?

This is a bug introduced with iOS 14. I am currently running iOS 14.5 beta and can report that the bug still persists. It affects both AirPods Pro and Max.

The thread above hints at a solution for AirPods Pro (change settings with case open and APPs docked in it) but that didn’t work for me or many others. I like to run my AirPods with noise cancelling = OFF sometimes so I had to resort to using a shortcut to enable this setting, as this bug basically “robs me“ from the OFF setting. Frustrating!

Or thanks for that, appreciate the information and the link. I thought I was going mad ?.

I'll give the AirPods Pro solution (well for some) and see how it goes on there. In the meantime I can ask Siri to turn off noise cancellation as a workaround.

It seems to be taking a long time to fix it, as the issue started early December (from the thread you linked).
