I have iOS 15.0.1. In my contacts list I have “Emergency Bypass On” selected for “Text Tone“ for a certain contact, but when I receive a text message from them my iPhone does not vibrate when Do Not Disturb is enabled.
 
Under Focus/Do not Disturb - People - add whoever you want from contacts.
 
I was playing around with my settings to see if I could find an answer for you, but discovered I have the opposite problem. Also iOS 15.0.1. I have Do Not Disturb on with NO people/app exceptions, yet when I called my phone from another line just now, the phone rang loud and clear as if I didn't have Do Not Disturb on at all! Not cool!

There used to be an option (that Apple appears to have removed or possibly buried elsewhere in the OS) that let you choose whether you wanted to always silence notifications when Do Not Disturb is on, or only when the phone is locked. So I made sure my phone was locked when I called it, yet it still rang.

The 15.0.2 update is out, so I will update my iPhone tonight and see if it's fixed.
 
I also did that, and the problem is not resolved.
I have DND or focus automatically activated when I launch an activity or workout. All notifications are silenced except from my wife who I added in that section from contacts. Hers are the only texts or calls that come through when I’m doing a workout. So the feature does work. You may need to restart your device or try setting that up again. That is the setting for it.

I actually did not have emergency bypass on for text tone for my wife, only for ringtone, but her texts come through even when DND is activated.
