When I have firefox open for example and then press Command+Alt, I see a list of all open apps, and many times when I choose another app, the screen stays on the same apps and doesn't switch to the selected app. Why? And is there a fix please?

Do you mean CMD+TAB by any chance?

What's CMD?

I mean I press and hold down the key that says "command", which is next to option and space bar, then press TAB, I see all apps open and can switch between them, but most of the time it doesn't switch from firefox for example to the app I select.

Anyone know why and if there is a fix?

CMD is short for COMMAND. Different countries have different prints on that key. I just asked, as you said ALT + Command.

Anyway, which exact Mac OS X version do you use? 10.8.3, 10.8.4 or 10.8.5? Does it happen with all applications or only with some specific ones?
Do you use multiple Spaces or Desktops

Happens with most applications. Running 10.8 now, but happened on 10.9 too.

Don't think I use Spaces or Desktops.
