Something like that happend to my Bro's iPhone, but its jailbroken, and it happend because he download something from cydia that enable you to download files from the Safari

yes my phone is jailbroken.

any ideas?

well, did you read my post!?, there a app, or thing , cause it not a visual app (i doesnt add any icons) , that allow you to download files from safary so, everytime you go to see a video or something it will ask that (what the pic you post shows), i know my bro download it from cydia, then i had to see what was happening, and i found it (dont remember the name) and uninstall it, and problem solved.

Check that.
