Is there a way to start an Apple ID & iCloud email account at the same time so you only have one email address???

Yes. For new users with no AppleID (iTunes), when you create an AppleID, it will be your iCloud email as well. So, your AppleID, iCloud email, and iTunes account will be this account.

Sadly, if you already have an AppleID (iTunes for example), there is no way to link\merge them. You will have two AppleID, two billing accounts... Else, if you create the iCloud account by initially setting it up using your iTunes AppleID, you will not have an iCloud email address.

Good to know, but how do I set that up. When I go to set up the account it is already asking me for an email address which would negate the fact of having one @ iCloud....

One way is to go to iCloud.com on the web, and click the link to create a new iCloud account.
