I've got my first car with CarPlay (USB only) but now have a problem - I always forget the phone in the car, still connected with the USB cable. Our second car (also CarPlay with cable) warns me when phone is connected and ignition turned off with voice prompt - "your cell phone is still in the car", very convenient.
I have tried finding an app that could alert me when CarPlay is disabled (i.e. when ignition is turned off) but haven't found anything. Maybe an automation is possible?
Anyone have an idea?

If you get an Apple Watch it integrates with Find My and will notify you if you leave your phome like that.

The problem with Find My is there's a significant delay before the notification, often several minutes. If you have an Apple Watch, the app Phone Buddy with notify you with sound and vibration as soon as you're out of Bluetooth range, typically 30-50 feet.

I have a ~$30 wallet tracker card that works with FindMy that will do the same thing, just for my wallet. But OP was inquiring about an app. ;)

Maybe an Automation.

"When CarPlay disconnects" is an option/trigger in Shortcuts' Automations. Maybe that with a simple Notification.

Or play a sound. Playing a tone gets a little more complex (have not done this myself), requires getting a tone and passing it to the "Play Sound" Action.

But this all assumes phone speaker is not muted, set to zero volume.
