I have .mp4s that will play in iTunes just fine, but for some reason the Apple TV will not play them. If I try to re-encode them via Handbrake, Apple TV will not play them. If I try to convert them via iTunes; all I get is white video while the audio of the movie plays. It seems to happen randomly. Has anyone experienced issues like this? I appreciate any suggestions.

Re-encode them using what settings in Handbrake?

iTunes will play lots of things on a PC/Mac which won't play on AppleTV, its fairly easy to find something outside of spec in a .mp4 that breaks AppleTV compatibility.

If re-encoding using the AppleTV preset in Handbrake gives you a file that still plays on a computer but won't play on AppleTV I'd be surprised though.

I have tried changing the file name from .mp4 to .m4v but that does not fix the issue.

I am encoding my files using the Apple TV preset on Handbrake. The only difference is that I bump up the constant quality to 67.17%. It seems to work for most of my encodes. Why it doesn't work for all boggles my mind.

The file I am trying to encode is already an .mp4. It won't play on Apple TV. It has a size of 2.18 gb. The bit rate of the file is 319 kbps and is 1280x688. The codec is already h.264. It's just odd. Handbrake won't convert, iTunes won't convert.

No, those specs are "just odd". You have a 2.18 GB mp4 that has only a 319 kbps bitrate ??? This must be a 24 hour movie or something. Clearly a typo or a *very* strange file.

dynaflash will know better than I but isn't the resolution potentially a problem? Whats the FPS of the file?

And I didn't think the AppleTV preset let files goto 1280 wide for that reason? or am I imagining that.
