AppleCare+ $9.99/month. If Apple replaces a drowned iPhone with a refurbished iPhone, is the refurbished phone still covered?(as long as the customer keeps on paying the $9.99 every month)

Yes, as long you continue paying the $9.99/month fee you're still covered with the replacement device. It's when you cancel the subscription that you will no longer be covered.

what about the opposite? what if I immediately cancel the AppleCare+ after receiving the replacement refurbished phone? (for example, trading it it for a newer iPhone next month) :)

I dont think you will still be covered, a 1 year limited warranty should be added from apple but it is not.
