Wondering where the issue lies. Because at exactly 1/4th, it's so low, I can't hear anything.

edit: 1/4th volume, not headphone inserted

Does it work fine when you are at 1/3 or 1/2?

Good movies and music tend to mastered fairly quiet overall, so that the louder stuff is louder and the quieter stuff is quiet. This is arguably a feature, rather than a bug.

That's kinda a strange 'feature' since I've been having low volume problems since the latter part of the iOS 9 cycle till now (10.0.1). This has been happening on my iPad Air and 6S. No headphones, full volume, low audio. Wired headphones, full volume, still low audio. It's about half of what it used to be. Luckily, Infuse has a volume booster (even though it makes it 'tinny'). What's weird is that my BT headphones do a pretty good job—probably has a built amp. <shrugs>

It just goes from being loud, to quiet rather quickly. the increments in volume are to big compared to other OS releases, IMO... at 1/4th volume, NOT being able to hear anything is rather disappointing.

Yep. I am having the same problem. It is annoying. I have to crank up the volume in my car in order to hear at a normal level. Hopefully this is fixed in 10.0.2.
