Since updating to 10.0.2 i have noticed that randomly,although i have this option enabled for select apps,i find the option enabled for all my apps..

Does this happen to anyone else?

Yes there is a bug that causes this, I've seen it myself.

How to avoid it, .. after altering any background app refresh settings close the settings app down completely, do not leave it open in the phones memory and then go back to it later on.

If installing a new app after enabling restrictions, will the app be declined BAR or will you need to turn off restrictions to disable that app?
The newly installed app will be decline BAR straightway. There is no even a pop up.

The pop up saying if you allow the app to continue run in the background.
I haven't seen such for a year or so, therefore maybe I am wrong regarding what pop up says.
I'm not sure I remember one even. I think I recall one for location services use, but can't recall one related to BAR (although I might be forgetting something about it).

I normally just disable Background refresh on just per app, but a turn it off completely..
