13", 2019 model. Have a couple of months left on AC+, wondering if I could bring it in for a battery replacement.

Is battery condition not necessarily tied to battery health? Or is cycle count part of the equation too? I've had 1700 cycles in close to three years.

Does battery condition have to say 'Replace Soon' to be eligible for a free replacement, or is 76% enough?

As jav said, take it to a brick-n-mortar Apple Store.
NOT "a 3rd party provider" -- take it to APPLE.

Even if you have to pay (under AC+), it's still worth doing.

In the USA, the charge is $199 (without AC+).

At 1700 cycles, Apple isn't likely to replace the battery for free. This Apple document lists what Apple considers the full lifespan of the battery, which is 1000 cycles for all recent laptops. The battery is designed to hold 80% capacity for 1000 cycles, so if you're at 76% after 1700 cycles, you're way ahead of intended life.
