Has anyone else noticed that Safari eventually (if left running for several hours or a couple of days) will no longer show the icons when you tap "+" on the tab row to open a new tab? The Favorites that are all pinned there will all have blank gray icons instead of their normal icons based on each website. Killing Safari and reloading it solves the problem.

Unit seeing the issue is an 11" iPad Pro M2 with 16GB/1TB so space and RAM definitely isn't the problem (unless having too much is somehow a problem)
 
This just happened to me on Saturday on my iPP 12.9 M1. I don't know if it's because it's been running for a long time because I never close Safari but like you said killing it and reloading sovled it.
