Every now and then, it seems that Build and Run will build without error, but then start running the older version of the app that was previously on the device.

Has anyone else seen this?

Any idea why when Build and Run usually installs the current build, sometimes it doesn't? Phase of the moon?

yeah, it has happened to me sometimes. Just try cleaning the project. If you want to take it one step further, go into your simulator or device, go into edit mode (by holding down on an icon until everything wiggles) and hit the X to delete.

Yes, this happens. Just (right-menu) Touch any source file and rebuild, and it'll usually install.
