Calendar seems to lose appointments

I seem to have a problem of that sometimes I enter items into my calendar on my iPhone and then they later completely vanish. I also access calendar from my iPad and MacBook. The problem is possibly more frequent with appointments entered into the past. For example, sometimes I enter something after it has taken place just so in the future I can go back and see when the last time I did it was.

Does anyone know what possible explanations for this might be? Is it conceivable my iPad is overwriting the calendar on my iPhone or something like that?

I ran into something like this (a while back) and it turned out to be an issue with the multiple calendars. My Exchange calendar is primary and I was having an issue with my iOS calendar. As I use Exchange and Google (Exchange for Work, Google for personal), I turned off the iCloud calendar and my issue stopped. Never did find out why that appeared to be the problem.
Running 7 calendars on my iPhone, 4 on my iPad, and 3 on my rMB. The calendars are common on the iPhone.

I ran into something like this (a while back) and it turned out to be an issue with the multiple calendars. My Exchange calendar is primary and I was having an issue with my iOS calendar. What you might try is turning off a calendar and seeing if not solves the problem. You could temporarily use another app for that calendar.
Google - not a likely candidate but there have been crazier inconsistencies in iOS.
