I couldn't really find any sort of integration of that kind (except syncing your Freeform documents over iCloud), but maybe I am overseeing something.

I think it would be great if it was possible to draw something on the iPad, and have it appear in real time on the Mac.

I know this from other Apple apps such as Logic Pro/GarageBand, where you can work on your Mac but make real-time changes to controls on your iPad.

Does anyone know if this is possible with Freeform?
 
You can use macOS Sidecar feature to extend the Mac screen to the iPad.
But Freeform is made to be used by multiple users simultaneously, so sync via iCloud should be quite fast too.

If you open the same file on iMac and iPad, and draw on the iPad you will see the changes reflected on the iMac. However, it is not in real time. There's a lag of about a second or so.

You are right, it’s amazing! The small lag is negligible. I’m surprised it’s so fast.
