You don't, you just insert the both the sim unlock card and the sim card, and it should work. When I did that it displays t-mobile at the top like it should and I have full bars. Sorry if that wasn't the answer you were looking for, I'm confused as to what you are asking.
Nooo, that's not the answer 
When you go in Settings, you should see an option "Carrier" (I think it is below Wi-Fi). And my iPhone doesn't display this option.. I've seen a solution to this long time ago and can't find it with google, so posted here..
