I am currently in the Apple Upgrade Program and got my pre-approval thinking I would reserve the regular iPhone 12 Pro on Friday. I have changed my mind and decided I want to get the iPhone 12 Pro Max instead, which becomes available to reserve on November 6.

What happens now?

Will I have to re-apply for the AUP to make the reservation in November? Will they do a hard pull on my credit once again?

The email you got confirming your pre-approval states that you can do this.

"If you would like to change the model you selected or your AppleCare+ coverage, you’ll be able to do so when pre-order starts. Just keep in mind that this could affect your delivery estimate."

Thank you.

The part that confuses me is "This preapproval will last for three days after pre-order begins." My preapproval was for the October 16, yet pre-order for the iPhone 12 Pro Max does not begin until November 6.

Ah, now I understand. I hadn't thought about the implications of the different dates. It does seem as though you'd have to redo the preapproval... might be worth reaching out, however as the preapproval isn't directly through Apple I'm guessing they don't have any control of that.
