Is there a simple way to combine multiple MP3 files into a single file (ideally with chapter markers where each individual file starts/ends)? It seems like it should be easy enough but I'm struggling to find an efficient way. Thanks for any suggestions!

I am pretty sure Fission has that functionality. $30 but it is a solid piece of software!

In the past, I've used Sony SoundForge Pro for Windows, and I'm pretty sure that it's available for Mac (but it will leave a hole in your pocket, sadly)

... and yes, it works like magic ! Fantastic software for any kind of mp3 manipulation.
