iOS Confused by bank information in iTunes Connect

Hi folks, while developing my application I found lots of great info here and I'm hoping someone here can help me now that I'm ready to submit the app.

I'm currently quite confused trying to fill out the banking information for a paid contract. The first thing it asks for is "Your bank's address". When I click "add address" it states that PO Boxes are not allowed. The mailing address listed on Sovereign's site has a PO Box. I believe that is Corporate HQ. Do they actually just want the street address of my local Sovereign Branch?

If I just pick any old address I can see the second screen which only raises more questions. I have a normal checking account with Sovereign, so I understand the Account # and Routing # fields. I don't know of anything to put in Account Name or Branch/Branch ID. I also don't know the SWIFT code, but it sounds like I can call Sovereign for that information.

For anyone who has successfully completed this process, what do you put in those fields?

I used the address of the branch my account is held at. The branch ID, SWIFT code etc were provided by my bank.

Thanks! That was quite quick. I guess it's time for another phone call. I never thought developing and selling an little application would involve so many phone calls.
