I reinstalled OS X El Capitan on my Macbook Pro and now continuity only works for incoming calls, I can't use it for out going calls. On my iPhone 6 (iOS 9) I still have my old MacBook Pro install listed as a continuity device in addition to my new Macbook Pro install. I am suspecting that this is the root of my problem - my iPhone sees 2 Macbook Pro's. Any idea about how I could reset my available continuity devices? Continuity still works perfectly on my iPad Air 2.
 
After changing my Apple ID password my Macbook Pro is listed only once. Continuity still does not work for out going calls. Does any one else have this problem?

Edit: Continuity works from FaceTime, but it doesn't work from PhoneDock or Contacts

I'm having the same issue across all my machines. Incoming calls work with zero issues, can't make an outgoing call.
 
When I attempt to place a call on my Mac I receive the notification 'Your iPhone and Mac must be on the same network' or something like that. I also get the error of just 'Call Failed'. I am guessing it's just a bug - report it and for now we'll have to just make do :(.

I think there is generally an issue with continuity for me anyway - when taking calls on my Watch the Watch just hands off to my phone automatically which is very annoying. Thats a different topic anyway.
