Hi guys
Today I got woken up by my Mac Pro 5,1 around 6am as it woke from sleep and the intake fan went on at full blast.

I tried resetting the PRAM and SMC and eventually went ahead and connected the fan to the exhaust connector to make sure it's the fan and it certainly is. After connecting, it just blasts at full speed - MacsFanControl does not detect its speed. The exhaust fan works fine when connected to the intake connector.

Did it just die like that? Is there anything I can try to fix it or should I go ahead and just order a new one?

As you reverse tested it a new fan will cure it. If you like replace the outtake fan as well. Bearings will tend to rattle for the outtake fan.

Yeah, I will order one. It's about £20 on eBay for the whole front module with the speaker.
I will also take a look at the outtake fan to see if it rattles.

P.S. Gosh, I read your signature and I no longer feel bad for buying an iPhone 12 to play with and holding on to all that tech junk I have :)
