The iPhone turns up its radio transmitter power if it is getting a weak signal from a 3G tower, wifi access point OR Bluetooth device.

I found that if I leave my Bluetooth headset in the car outside turned on, it just kills my iPhone 3GS's battery life overnight, whereas if I put the Bluetooth device next to my iPhone, or turn off Bluetooth on the iPhone if I've left the headset outside, the iPhone's battery life barely goes down overnight.
