Prior to iCloud, I had/have a MS Exchange account set up on my iPhone. I use the exchange email and calendar. Now, I've also enabled the icloud calendar:

however, when I add an event on my iPhone, it doesn't show up on icloud.com's calendar. Is there a way to sync both calendars on the phone?

Also, how does ios5 determine which calendar is the "default" one (the calendar that is updated when you add an event from your phone)? Thanks for any help!

Go to Settings > Mail, Contacts, Calendar > scroll down to Calendars > select Default Calendar > choose which you'd like.

There is a default calender setting in Settings->Mail,Contacts,Calendars which by default creates the event for that chosen calendar. When you add an event you can select which calendar it should be created on. Your exchange calendar will only sync with that calendar...... you would have to create the event on both exchange and iCloud if you want them on both.
