I have the drive pulse feature enabled which monitors the health of the internal hard drive in Drive Genius and I have been getting prompts almost daily with an integrity warning, even after I wiped the drive, defraged and brought everything back over with carbon copy cloner.
But when I run the apple hardware test at bootup, everything passes? How reliable is the Apple Hardware Test in diagnosing a hard drive issue?

AHT ranges from "extremely unhelpful" to "I shouldn't have bothered". It's basically a GUI older than the sun that reassures you your computer isn't completely fried, as at it'll show something when it turns on. Beyond that, it's no good – not even Apple use those diagnostics.

If third party tools throw up SMART errors, that's almost always the case. I'd replace the drive.
