Hi I am thinking about buying a second ATI 5770 is it plug and play.
I have a Mac pro 5.1
Can I hookup a third monitor to the second card?

Yes, but in theory, if you use 2 true displayport displays and one dvi you should hook up three displays on one 5770.

or instead of real displayport active DisplayPort adapters.

I previously had dual 5770 cards before my RX580, worked great.
It means you can have dedicated gpu per screen (I.e. play a video on one screen, the other one will not be affected at all, whereas with a single gpu/screen playing a video would make other tasks laggy).
I had one monitor on one card and two on the other (one was actually a rift, but it’s classed as a display).

Why do it? Unless you are locked to an older macOS release than Sierra because a specific software that you need to run, don't make sense to buy a second ATI HD 5770 today. It's a card that is supported up to High Sierra only and High Sierra supports ends the day Apple releases Big Sur, any day now.

If you want to have Mojave support you need a newer GPU, even a mid-level modern GPU like RX 560 fully supports 3 displays nowadays.
