Duplicate Application Images

I downloaded a few apps tonight and everything was fine until i noticed two of my apps had duplicate images.

I did a hard reset and the problem happened even more..

Anyone know if this is a bug and how to fix it?

I have an iPhone 3GS Black 32GB.
