So my Mac is showing this update twice! It says it is updated to OS 10.11.4 which I can verify when I check on "About this mac" so My Mac says it´s already installed but when I click on the update that keeps showing it installs an app on my launch pad that wants to install the update again. Any clue why is this happening and if I should indeed install the update again?

That looks like an app update for the installer rather than an OS update. Delete "Install OS X El Capitan" from Launchpad and refresh the App Store, the update should disappear. You can re-download it from App Store > Purchased if you still want the installer app.

Software update has been showing these kind of errors several years. I first reported similar error when Mavericks was released and for some reason Apple still hasn't fixed it.

If your Mac is otherwise behaving normally there should be nothing to worry about. Just hide the update, click on the update with second mouse button and select "hide".

Maybe Apple fixes the software update in a future update, maybe not...
