I'd like to setup my gmail to forward mail to my iCloud mailbox on my mac. Is it possible to do this but also have the ability to reply from my gmail address from the mail app?

Why not just set up both accounts in Mail and then when Gmail emails come in, you can reply directly to them without the need to set up forwarding and reply-to addressing?

Re: reply with different address yes.

As link above, setup the accounts in Mail. When replying, in the "From" box, select the account you want to reply from.

I'd just forget about forwarding, setup each account in Mail, and then do above. Will have the advantage of keeping e-mails organized by service they come in from. Less clutter in whatever account is considered "primary".

Ok thanks, I tried setting it up as per the link but it would just reply from my iCloud email rather than my gmail when the gmail was selected from the drop down box.

Works for me with Gmail and iCloud.

Check to see that you have smtp servers setup for each of the services. And, maybe, have the "always use this server" checked for each server (with appropriate server selected).
