I talk often to a very hard of hearing friend and I have to practically shout in order for her to hear me when I'm wearing my earbuds. I wear them because she talks a long time and I can work around the house while chatting. By the time we get through I'm practically hoarse from having to talk so loud. Is there an iphone app that will boost my voice or is there a better set of headphones that I can use with a better mic?

Why can't she get the hearing aids that connect to the iPhone? My cousin has it. He loves them.

Does your friend use a headset? If not, suggest she get one and use it.

I'm also going slightly deaf, and I find even the Apple headset to be a big improvement over talking on the phone itself, especially in noisy environments (like walking down the street). A noise blocking headset would be even better, but I like to be able to hear outside sounds (like vehicles when outside, or coworkers when in the office).

She is 96 and refuses to use a headset or smart phone, unfortunately.

No app I'm aware of. You might consider a Bluetooth headset.
Would a Bluetooth headset's mic be that much more superior to the built-in mic in the Apple earbuds?
