Hi, I have a MacPro5,1 (Mid 2012) flashed with the latest
modified with enableGOP 1.4 although this issue has been persistent even prior, just not pronounced as I'd been running a non-EFI RX480
and whenever any of the installers reboot or I attempt to do a startup disk reboot, I get the error logo and the machine halts all processes. It isn't particularly an issue as I can hard reset the machine and it starts up as expected this time, however I am concerned about damaging my disks in the process. This only happens on reboots, if I shutdown the machine and start it back up, everything functions correctly. I am confident in the integrity of my NVRAM as garbage collection appears to function correctly, It's recently been reflashed with a
I did 5 consecutive NVRAM clears before immediately taking a dump
bootROM. Macschrauber's dumptool also tests OK. Has anyone come up with a solution or is aware of what the issue may be? Thanks!
 
Could it be that you're having what I have? For me it happens every 2nd or 3rd boot and then machine keeps rebooting after halt at this page (although it restarts gracefully, it syncs disks etc.):

After I do pram reset (three times) then it boots fine past the same error:

I had Monterey which I upgraded to Ventura and after the upgrade this started to happen. @Macschrauber dump tool says OK although its possible that my nvram is messed up (I plan to fix it soon with @tsialex)

what drives are in?

For example Samsung 870 EVO 1TB+ are known to be troublesome on warm start. They simply do not mount.
 
Thank you for your response. I have Samsung SSD 970 EVO Plus 500GB, APFS trim disabled in OC. I have flashed 4,1 > 5.1 Mac Pro dual CPU. Whats interesting it worked without issues with Monterey, then after upgrading to Ventura it started to have this issue every 2-3 boot.

update: I'm suspecting SATA issues as I have 3x SATA drives as well, per tsialex great advice!

Tried with m.2 only, but it still happened. Got fed up since I believe it was messed up when upgradign from Monterey to Ventura, so I did clean install Sonoma today and it works great. On to nvram chip fixing with tsialex now! Thanks for your help (also wonderful tools @Macschrauber you have!).
