Does anyone know of a fish eye add on to use on the iPhone video camera?

If you are looking for a JB tweak then no there isn't because taking fish eye photos requires specific lens specs (eg. Canon's EF 8-15mm f/4L).

If you are looking for a software that does this effect then there maybe. Or you can use photoshop to edited when you import the photo.

If you are looking for an accessory then this should be in the accessory section.

You cannot get the fisheye lens without an actual hardware fisheye lens. You can edit it to add distortion, but it simply looks strange because the field of view is off (fish eye lens' will simulate roughly human eyesight, 170º field of view). Your average camera, with a focal length of 18mm will have a field of view of around 65º. There simply is no way to 'edit' field of view.
