It is just a disk image that was opened and used a history listing if you will, to get rid of the idea uninstall flash or set it to not update automatically. Really better off getting rid of and just use Google Chrome for sites that require flash and will not serve up HTML5 instead in its place.

Maybe its just a disk image installer file for flash left inadvertently on the desktop or somewhere.

i can eject the 'flash player' part, but the disk image remains...yet i don't see it ANYWHERE on my mac. how can i kill it completely?
It's not actually still on the Mac. Just right click on it and choose "remove."
