iPhone Folder/Switcher Theme Issue

I went to install Phone GV Extension in Cydia, restarted Springboard and this happened.

I've tried all of the following with no luck in getting it fixed:
- Uninstalled Phone GV Extension
- Checking/Unchecking all themes within Winterboard
- Safe Mode
- Rebooting
- Hard Reset
- Deleting switcherlinen.x from caches

Anyone have any other suggestions?

I don't see where is says it supports 5.1.1 only up to 5.0. I would report it to the Dev so he can maybe fix it. his twitter is @gvexts or website http://gvexts.appspot.com/

Already uninstalled and it still didn't fix the issue.
