I ran a test on the small, medium, and large tips. All three results in a good test in both ears.

How do I know which size I should really use? I'm debating between small/medium. What would be some potential differences if I went with one over the other?

similar boat and wondering if the slightly larger ones will sound better due to a better seal

Same experience. Found the larger tips to offer better noise cancellation. However comfort levels over longer periods may determine which you opt for in the long run

Put the APPs in your ears, enable noise cancellation somewhere where you can hear it filter out background noise and open your mouth really wide like you were yawning. Repeat with all tip sizes in both ears.

If you can hear the background noise return while your mouth's wide open that size is not providing a solid seal. Find the most comfortable size from the ones that don't fail.

but all else equal, if both fit fine, the medium size will produce better sound than the small size?

I am using the large one on my right ear and the medium one on my left ear. For some reason the large one didn’t hold as well on my left ear and I could feel the AirPod slightly moving a little in my ear
