The iPhone automatically backed up every time I synched it but now when I did a full restore it only gave me about a dozen apps backs. Almost none of my paid apps.

How do I get them back? Am I just screwed and out 100$ in apps?

If you haven't backed up the original .ipa files (always a good idea!) and/or you don't have them in iTunes, you can re-download them from the App Store. You WILL NOT be charged again for doing so.
 
How do you do that, just "buy" the app again and they won´t charge you?
Yes. You click buy and eventually it'll tell you that you have already purchased the app and ask if you want to download it again (no charge).
