I have a 38mm SS apple watch. I got a good deal on the Milanese band but find that while it wraps around my wrist I can not fit my fist through the loop when it is attached to the watch. Is there a way to get the end of the band to slide through the adapter lug?

Yes I've been practicing that and I can just fit it through now so that's good. I just wish there was a way to get the strap to slide out of the lug. does it wear down enough with use?

I think it's a feature. This way you are less likely to have the watch catch on something and call to the ground.
yin & yang one mans bug is another mans feature.

No. It will not fit through the lug.

You could always disconnect one end from the watch, put it on, then slide the lug back in to the watch. Probably not easy to take off this way, though.

Yup. This alone is why I'd rather use the Milanese loop than the leather loop. One of my coworkers said that his leather loop came undone a couple times when playing with his little kids and the watch dropped to the floor.

I tried that and the problem you get then is trying to get your finger under the band to reach the release button. after some practice I have been able to squeeze my hand enough to work the band on and off.
I was wondering if there was any hope of using a small flat file but from what I can see the opening actually narrows to catch the strap end.

Sounds like the ML band is not for you. Large hand and very small wrist...
You could always try for a 3rd party mesh band with a conventional clasp that separates the band in half.

Sounds like an assumption on your part. The 38 with a protective case on it is as large as I want and I have a normal wrist of 8inches. As I said I can fit it on so it's not a real problem, also I got it for $30 off. I like the Milanese band for evening events so it's for me since I think the link bands look cheap.. The black plastic band fits fine and I now have a Synergy NATO style nylon for the day and that fits just fine also. So it's all good.

Well that's not helpful at all it sounds like you just want to be sarcastic and mean. You are not even correct as the size of the hand is determined by bone not fat.
