I understand they work very differently and understand the differences but I mean do they both do the same thing in terms of their effects on the screen?

No. Tru- Tone uses the ambient sensors on the iPhone to correct the white point on the display to make reading more natural based on the lighting conditions in the room.

Where as Night Shift simply turns the display yellow at a specific time and is controllable to give it a warmer effect over the blue light spectrum exhibited.
