How do I add a poll to my website?

You didn't mention your scripting language and database format, so here are some common but useful links to help you, generally speaking:

The first choice is by far the easiest to implement. The others get progressively harder. Note, there are literally a million ways to do a poll, wait for others to reply here with ideas before deciding.
