I turned on two factor authentication with Apple about a month ago, with no issues. But now the only options to send a verification code too are my phone number, which is fine, or my iPhone 5. I have an iPhone 6 now though. So before I get rid of the iPhone 5, how to I remove that from my list of trusted devices, and add my iPhone 6? (and if possible, I'd like to add my iPad 2 as well). Any help is appreciated!

Sign in with your Apple ID on appleid.apple.com. Choose "Password and Security". Choose "Manage your trusted devices".
