How to rate apps?

Hey,

Stupid question, I know, but how do I rate an app? Every time that I rate an app from my phone the comment is not visible after the refresh. Do you need to create some type of special username in order to leave comments?

What I meant was why do these people have usernames when they leave comments? I do not have a username and have to log on to iTunes using my email. If you look at the screenshot, it doesn't appear that the raters are logging in using their email addresses.
