I was trying to transfer over from T-Mobile's family plan to AT&T's family plan with one iPhone. On Saturday, the rep told me I MUST open as individual plan for each line because there is no 'Family Plan with ONE iPhone' option in iTune. He said I can easily combined the accounts together after I received SIM cards for other numbers/lines. Today I gave AT&T another call, they told me my account is locked for 30 days because these accounts are new!! That means I'll pay each line as individual plan for first month. AT&T said this is the way it has to be for all other ppl transfering into Family Plan with iPhone. Did anyone of you having the same issue of not able to sign up as family plan?

That is nonsense.

I was already a AT&T Customer on the family plan, but i could add just one iPhone. Whoever told you, that you can't have one iPhone on the family plan is flat our wrong. You are going to have to keep on them until you find someone who can fix this for you.
