how many folders named Pictures does iMAC for desktop actually come with when you purchase a new desktop?

As far as I know, macOS creates one Pictures folder for each user. Doesn't matter whether on a desktop (iMac) or laptop (MacBook).
There may be other subfolders that happen to be called "Pictures" for various apps, such as under Photobooth, but these are not the same thing.
Not sure why you are asking, but if it is because you don't see the Pictures folder in the Finder sidebar, you can change what shows in the sidebar by going to:
Finder
Preferences
Sidebar tab
